CLU Athlete:
Steps to follow to get cleared to participate:
1. Print the Pre-Participation Physical Evaluation, fill it out completely, take to your doctor for a physical.
2. Sign in to the Student Health Portal. Click on "Document Upload" and select "Athletic Forms". Upload a) your COMPLETED forms and b) a picture of the front and
back of your insurance card. You will receive a secure email after your form is reviewed by Health Services.
3. Make sure Health History Forms are completed and uploaded. (if not previously turned in to Health Services as a new student in your admissions packet)
